Who are you?

This information is needed to contact you and associate this request with your customer history.

License Type: Select whether you are using a free web trial, have a commercial license contract, or are a DEI sales partner.
Customer ID: If you have a contract with DEI, you will have been issued an 8 character Customer ID. Users of the free web trials may leave this blank.
Name / Title: Your name, as you prefer to be addressed, and your job title.
Company: The name of your company or the entity licensing the software.
Country: Your country.
Address: The address of you or your company. Use both lines if needed.
Email: Enter the email address where you wish to be contacted regarding this issue. Retype it to ensure accuracy as this will be our primary means of responding to your request.
Phone / Fax: Enter the voice and fax numbers where we may also contact you if necessary.
Prior Incident ID: If this TSR is in relation to a previous incident, include the ten character incident ID number here.
